At Reputable Home Health Agency we offer shift care, our Registered Nurses, LVNs and Home Health Aides provide exceptional care to patients in the comfort of their homes under the guidance of a physician.

Registred Nurse

At Reputable Home Health Agency, our Registered Nurses provide exceptional care to patients in the comfort of their homes under the guidance of a physician. They coordinate and direct patient care based on the patient's individual needs. They are responsible for the independent management of patients requiring the use of advanced assessment, teaching and decision-making skills. The Registered nurse is also responsible for ensuring that appropriate referrals to other services are made, and communicate with the patient’s doctor after each visit. Interdisciplinary conferencing takes place regularly and relevant knowledge and experience is consistently applied to our patient population.

Licensed Vocational Nurse

Our Licensed Vocational Nurses (LVNs) provide basic, bedside nursing care for sick, injured, convalescing, elderly, and physically or mentally disabled persons. They typically work in a health care team under the direction and supervision of a physician and registered nurse and develop their skills in a number of clinical areas. They observe and monitor patients, perform basic assessments, and report and document changes in a patient’s condition. They also measure vital signs, perform medical treatments, and administer prescribed medications. Licensed Vocational Nurses may collect samples, feed patients, and record food and fluid intake and output. Some care for, and feed newborn babies and toddlers. Licensed Vocational Nurses can also supervise certified nursing assistants and aides every 14 days.

Home Health Aides

Home Health Aides help elderly, convalescent, and the disabled to remain in their homes instead of a healthcare facility. They work for licensed home care agencies, such as Reputable Home Health to provide basic care under the supervision of a Registered Nurse or Licensed Vocational Nurse. As the number of people living into their eighties continues to grow, people prefer to stay at home where they are more independent, and comfortable, also the cost is usually lower than nursing home rates. Home Health Aides teach their patients ways to care for themselves despite illnesses or disabilities. They keep their patients mentally alert by talking and listening to them.

About our Nursing Care

Our highly skilled and compassionate Registered nurses and Licensed Vocational Nurses work together with our clients, family, and physician to develop an individualized plan of care to meet each patient’s unique needs. By providing clinical care, clients and family education, and support; our nurses coordinate a home care program to help clients and families become independent in managing their care safely at home.

Special Services

Assessing Vitals

Assessing vital signs including blood pressure, heart rate, respiration

Assess medications

Assess medications and side effects, ensuring appropriate medication management

Coordination

Coordinate care with all members of the patient’s healthcare team and arrange for additional services

Self-Care

Teach self-care techniques for all medical conditions including asthma, diabetes, stroke, heart and respiratory conditions, wounds, ostomies, catheters

Education

Provide education on safety, handwashing, ostomy care, suctioning, ventilator care, nutrition and dietary restrictions related to patient’s medical condition

Control Illnesses

Control the impact of acute and chronic illnesses such as diabetes, heart disease, respiratory illnesses as well as help reduce the complications caused by wounds and ostomies
